Which comes first, a physical or mental breakdown?

I still mostly stand behind my rankings, but after reading about Trump's eating habits and that he doesn't exercise or even sleep that much, the fact that he is the oldest person to ever become president, and the fact that his doctor is a quack I'm beginning to wonder if I should switch #3 and #4.
